We'll miss you, Dr. Cook!

Pediatric Health would like to thank Dr. Jason Cook for his hard work, compassion and the excellent care that he has given to our patients over the past couple of years. In early April Dr. Cook will be relocating to Minnesota to be closer to family. We wish him all the best and know that no matter where he goes, he will always touch the hearts and lives of everyone he meets.

For families who have Dr. Cook listed as your child�s pcp with your insurance carrier, you will need to choose another physician at this time. If you would like to continue to have your child seen here at Pediatric Health Associates, you may choose any of our physicians as each participate in the same health plans.

Jason, thank you for brightening our days! It�s been a pleasure and honor to work with you. We will miss you!
